Notices to Funded Agencies
Following are links to forms discussed in the August 2011 Accountability Workshop. Agencies may use their own forms, however please check to ensure that your forms include language similar to JWB and collect similar information. During annual program reviews, Contract Managers will be reviewing participant files to ensure compliance with Data Business Rules, documentation that participants have given the purpose statement for collecting social security number, release of information, and incident reports. Our Fiscal Analyst will be checking for completed Affidavit of Good Moral Character.
