PCHD - Special Teen Outreach Program
Special Teen Outreach Program (STOP) has trained care coordinators who are ready to help teens face the challenges and learn how to make informed decisions regarding developing healthy relationships, planning for education, sexual activity, good nutrition and regular physical activity.
STOP provides a one-on-one intensive experience for the teen. The care coordinator works to develop a meaningful relationship so that she becomes a trusted adult who acts as a mentor and a coach. Each teen is asked to make a commitment to meet with her care coordinator at least three times.
